Media Consent Form

I assign the copyright of media output from this interview featuring me including any audio recordings, transcripts, photographic or motion image or any mobile app submitted images, video or audio recordings (now or hereafter known Materials) to Chitchat's client, its client and its client’s Related Bodies Corporate (as defined in the Corporations Act 2001 (Cth) (together being the ‘Client’).

I grant Chitchat's client and its Client the unrestricted rights and permission to record, copy, edit, adapt, modify, distribute or exhibit in whole or in part, the Materials for any purpose whatsoever, including marketing, advertising or public relations (this includes the use of Materials across social media platforms) and for public or private use.

I release to Chitchat's client and its Client all rights to exhibit the Material in print and electronic form publicly or privately and waive any rights, claims or interest I may have to control the use of my identity or likeness in the Materials and agree that any uses described herein may be made without compensation or additional consideration of me.

I release Chitchat's client and its Client (jointly and severally) from all claims and liability relating to the use of the said media and indemnify Chitchat's client and its Client (jointly and severally) against any and all liability howsoever for all actions, proceedings, claims, damages, and expenses arising out of any breach or repudiation of this release by me.

Voice recordings

Photographs

Video recordings

I represent that I am at least 18 years of age, have read and understand the foregoing statement, and am competent to execute this agreement.
